Anyone may receive care at our clinic, simply by scheduling an appointment at (701) 234-9400. The most seamless way, however, is to have your medical records transferred to our clinic ahead of time. To do this, simply contact your previous provider and request a release form. Signing a release form will transfer your complete medical records to Independent Family Doctors. We also recommend that you contact your insurance company to notify them of the change.
